If you live in Tryon, Columbus, Saluda, Mill Spring or Lynn, you know the problem: there are not many therapists nearby. When we searched in September 2026, we found few practices with their own website and an office inside Polk County. Several local clinicians appear only on directory sites, so we left them off rather than guess at what they offer. They may still be worth a call.
What follows is what we could verify: two community clinics and a part-time private office in Columbus, an addiction treatment center in Mill Spring, and two practices a short drive away. Below the list are the public route through Vaya Health and advice for choosing when options are thin.
If you are in crisis now, call or text 988 (the Suicide and Crisis Lifeline). If anyone is in immediate danger, call 911.
How we chose
- The practice has its own website describing what it offers, and we could open it in September 2026.
- It sees people in Polk County, or it is one of the nearest options outside the county and we name its town.
- It publishes who it works with and what it treats.
- It publishes its licensing, accreditation or public funding status, so you can check it.

The honest picture: Polk County is short of therapists
Polk County is small and rural, and therapy offices are scattered. Many people in Tryon end up choosing between a community clinic, a part-time private office, a drive to Hendersonville or Spartanburg County, or video sessions with a clinician licensed in North Carolina. The best fit is the one you will keep going to.
The public option: Vaya Health
Polk County's public mental health system is managed by Vaya Health, the LME/MCO the NC Department of Health and Human Services lists for Polk. Vaya is a public managed care organization, not a clinic. It manages mental health, substance use, developmental disability and brain injury services for NC Medicaid members in its plans, and for uninsured people through state and local funding.
If you have Medicaid or no insurance, call Vaya Member and Recipient Services at 1-800-962-9003 to be connected with provider agencies, such as the clinics above. Its crisis line, 1-800-849-6127, is answered 24 hours a day.
Other local help
Steps to Hope in Columbus serves people affected by domestic violence and sexual assault, with counseling, a women's support group, emergency shelter, legal support and a 24-hour line at 828-894-2340.
How to choose when the options are thin
When you call, these questions help:
- Are you taking new clients, and how long is the wait?
- Which days are you in this office?
- Do you take my insurance or Medicaid, or offer a sliding scale?
- Do you work with my concern, and with couples or teens?
- Check any license with the NC Board of Licensed Clinical Mental Health Counselors or the NC Social Work Certification and Licensure Board.
